Supported Living and Healthcare Services Facilitator Role:
Mainstream Support, Inc (MSI). exists to provide services of uncommonly high quality to people with Intellectual and developmental disabilities (I/DD) and their families. Program:
Supported Living Services (SLS) assist individuals in developing a Life's Plan. This will likely include identifying and securing housing, building a support network of friends, family and employees and providing the support necessary for individuals to successfully manage their lives, to the greatest extent they can.
Purpose of Position:
The primary functions of the Supported Living and Healthcare Services Facilitator are: 1. Ensuring that Mainstream clients receive services of excellent quality to live with greater independence, choice, and a sense of belonging in their community and have supports tailored to their specific needs. 2. Building and maintaining a capable, high-performing SLS team through active staff supervision and development. 3. Responding immediately and appropriately to crisis situations or special incidents, as directed by the Healthcare Coordinator, including being on call to provide non-licensed support. 4. Providing required service to MSI's funding agent San Andreas Regional Center (SARC), which is responsible for case management and the funding of clients' services. 5. Expanding the network of community resources and support available to clients and promoting access to ensure greater independence, connection and interdependence. 6. Serving as a visible, professional representative of Mainstream's mission in the community and among referral sources, families, and key stakeholders to maintain and strengthen partnership. 7. Case Management coordination. 8. Providing administrative and logistical support to our Healthcare Coordinator within the scope of a non-licensed role. 9. Ensuring that all services and interactions with clients, families, and partners reflect Mainstream's standards of care and professionalism. 10. Strive to continually improve Mainstream's services.
Supported Living Facilitator Duties and Responsibilities:
May include but are not limited to:
- Coordinating the development and implementation of SLS program services and client support plans.
- Supervising Supported Living Staff at all stages of the employee life cycle - including recruiting, hiring, training and coaching, performance management and separation - in collaboration with the Human Resources Department as applicable.
- Assisting clients in securing a home of their choice.
- Assisting clients in identifying and accessing needed resources to enable them to live successfully in their community and build a sense of belonging.
- Advocating for clients' rights, preferences, and access to services in coordination with regional center staff and community partners.
- Monitoring client wellbeing and ensuring dignity while responding promptly to safety concerns or service gaps.
- Developing and maintaining liaisons with generic community resources which benefit the client.
- Supervising and coordinating any services conducted by other staff or agencies for a client.
- Facilitating circles of support as requested by client.
- Maintaining communication with San Andreas Regional Center staff to support client case coordination.
- Performing resource development and outreach to expand the network of community resources and supports available to the client.
- Designing and implementing client satisfaction procedures.
- Developing, implementing, and maintaining data and reporting procedures to ensure efficient work flow and communication.
- Completing other duties as assigned by supervisors.
Healthcare Services Facilitator Duties and Responsibilities:
Under the direction and supervision of Mainstream's Healthcare Services Coordinator (HSC), responsibilities may include but are not limited to:
- Coordinates and schedules physician/health care appointments for clients; collects after-visit notes, discharge summaries and referrals and routes them to the HSC.
- Ensures clients timely receive medical supplies and medications, including picking up and delivering medical supplies and medications to clients' homes.
- Collects, organizes, and maintains client medical records and documentation.
- Communicates pertinent client information to San Andreas Regional Service Coordinators and Health Service Professionals via health care reports, e-mail, fax, or phone.
- Assists in maintaining accurate medication lists, logs, and data sheets, updating as necessary, consistent with the scope of a non-licensed support role.
- Assists in gathering preliminary documentation and tracking deadlines related to special incident reports, supporting the HSC's timely completion in accordance with SARC guidelines.
- Develops positive, respectful working relationships with staff, community contacts, clients and their families.
- Performs other duties and administrative tasks as assigned by the agency director and/or HSC that are within the scope of a non-licensed support role.

Standards of Conduct and Performance:
The employee must maintain the confidentiality of protected client information in accordance with applicable laws and Mainstream policies. The employee is also expected to comply with Mainstream's goals, policies and procedures and to meet all performance standards based on Mainstream's established performance criteria. These expectations apply to all duties and functions of the position and are conditions of employment rather than individual job functions.
Qualifications/Education:

- Certification, training, coursework, or professional experience in healthcare, nursing assistance, medical administration, or a related field is strongly preferred.
- Excellent verbal and written communication skills, including proficiency in English.
- Proficient computer skills, including typing, data entry and Microsoft Office Suite for maintaining records, documentation and professional correspondence.
- Strong skills supervising others.
- Ability to accurately assess the quality of services provided.
- CPR/First Aid certification.
- A valid Driver's License and a clean driving record.
- The ability to pass a DOJ-level background check.
Reports to:
This position reports to the Managing Director while also maintaining daily communication and direction from the Healthcare Services Coordinator regarding medical support responsibilities.
Exempt Status:
In most cases this position is exempt under both state and federal laws.
Work Schedule:
Approximately 40 hours per week. Flexible and on-call hours as required by the client/staff and program, however, primarily Monday through Friday, 8am to 4:30pm. Employment with Mainstream is at will and may be terminated at any time by the employee or the employer, with or without cause.
