2996663178-2 Apply for this job now Job Description Hartford Healthcare is seeking a Quality Assurance Performance Improvement Coordinator RN to drive quality and patient safety within our HomeCare services. In this role, you will lead QA/PI initiatives, conduct chart audits, analyze clinical and operational data, and monitor key performance indicators to ensure compliance with regulatory and accreditation standards. You will collaborate with interdisciplinary teams to develop action plans, update policies, and provide staff education, supporting a culture of continuous improvement, teamwork, and compassionate, high-quality home-based care across our Connecticut network. Responsibilities Lead quality assurance and performance improvement initiatives for Home Care services Conduct clinical chart audits and reviews to identify gaps and trends Analyze quality and operational data to monitor key performance indicators Ensure compliance with CMS, Joint Commission, and state homecare regulations Develop and implement corrective action and performance improvement plans Collaborate with interdisciplinary teams to standardize best practices Provide staff education and feedback on quality and documentation standards Update and maintain QA/PI policies, procedures, and workflows Prepare quality reports and present findings to leadership Support a culture of patient safety, continuous improvement, and compassionate care Required Skills Quality assurance Performance improvement (PI) methodologies Clinical auditing Data analysis and reporting Root cause analysis Regulatory compliance (CMS, Joint Commission) Homecare clinical practice Electronic health records (EHR) Policy and procedure development Staff education and training
