Job Description

This position is for a skilled nursing facility under new ownership on June 1st, and has both short-term and long-term patients. This facility will have an in-house therapy team with smaller daily case loads, all new equipment, and an emphasis on optimizing time with patients. The Outpatient Therapy Coordinator is responsible for managing and supporting all aspects of outpatient therapy services within the facility. This person should be a licensed OT, PT, or ST. PTA and COTA applicants may be considered based on experience. This position includes coordinating referrals, managing intake documentation, ensuring payer verification, marketing services to referral sources, and maintaining a productive caseload. The Coordinator will serve as the liaison between therapy staff, community partners, and patient families to ensure high-quality, patient-centered care. Key Responsibilities:
Develop and maintain therapy marketing materials for hospitals, assisted living facilities (ALFs), independent living facilities (ILFs), and skilled nursing residents/families.
Include in materials:
accepted payers, therapy disciplines offered, frequency of services, specialized programming (e.g., Ultra Mist, Vital Stim for Dysphagia), and any specialized equipment for onsite services. Ensure the outpatient intake packet is complete and uploaded to PCC prior to the start of therapy services. Collect and scan copies of the patient's ID, Social Security Card (SSC), and insurance cards. Confirm payer source with the Business Office Manager (BOM) using a payer verification form. Discuss copayment responsibilities with patients or their representatives prior to treatment initiation. Coordinate with the DOR and Assistant DOR to schedule evaluations, ongoing treatment sessions, and follow-ups. Conduct weekly confirmation calls to outpatients to ensure adherence to scheduled sessions and provide reminders. Collaborate with community physicians and external therapy providers to facilitate patient registration and system access in PCC and Net Health. Maintain a healthy, interdisciplinary outpatient caseload across all therapy disciplines (PT, OT, ST). Monitor outpatient documentation timelines and ensure compliance with billing and clinical documentation standards. Completing scheduling and outpatient projections daily. Adjusting schedules for cancellations.
Preferred Qualifications:
Prior experience with outpatient therapy coordination or medical office coordination Therapist of any discipline
PT/OT/ST
Familiarity with Medicare, Medicaid, and commercial insurance plans
