The Role We are seeking a Transitional Care Coordinator to serve the Davis and Weber County areas.

This is a dual-focus role:

you will act as a professional advisor to families in transition while simultaneously serving as a community liaison, building vital partnerships with healthcare professionals and senior advocates throughout Northern Utah.

Key Responsibilities:

Senior Care Advisory & Advocacy Holistic Assessment:

Conduct comprehensive discovery sessions to evaluate a senior's medical, social, and financial requirements.

Strategic Consulting:

Provide families with a curated selection of senior living options (Assisted Living, Memory Care, etc.) based on our proprietary safety and quality data.

Tour Leadership:

Personally accompany families on tours of senior living communities, acting as their advocate and advisor to ensure all concerns are addressed.

Transition Oversight:

Coordinate with families and providers to ensure a seamless move-in experience and a successful long-term outcome. 2.

Marketing & Community Liaison Professional Networking:

Build and nurture referral relationships with hospital discharge planners, social workers, case managers, and elder law attorneys.

Brand Ambassadorship:

Educate the Davis/Weber community on the benefits of an advisory-led approach to senior care through presentations and local events.

Strategic Growth:

Manage a territory outreach plan to ensure CarePatrol remains the "gold standard" resource for healthcare providers in Northern Utah.

CRM Management:

Maintain diligent records of professional outreach and family progress to ensure high-touch service and follow-through.

Experience:

2+ years in healthcare marketing, senior living, hospice, or a related field. Professional experience in social work or healthcare navigation is highly valued.

Local Expertise:

Strong knowledge of the senior care landscape in Layton, Ogden, Kaysville, and surrounding areas.

Communication:

Exceptional ability to simplify complex information for families during high-stress situations. Requirements Reliable transportation and a valid driver's license for daily travel across Davis and Weber Counties. Self-motivated and capable of managing a field-based schedule.
