Full-time Transportation Driver SUMMARY:

The Transportation Driver will provide the transportation of patients to and from hospitals, medical offices, dialysis centers and private residences in a safe, secure and professional manner.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Transporting patients and clients utilizing the company vehicle in a safe and professional manner Assist patients and clients as needed to ensure safe transfer Effective communicate with facility regarding schedule progress and to receive additional instructions Understand how to operate GPS, Two-Way Radio and/or Cell Phone to navigate effectively Complete daily vehicle pre-trip and post-trip inspections, and maintain vehicle cleanliness Maintain accurate activity and vehicle report logs Other duties as assigned

QUALIFICATIONS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill, and/or ability required. High school diploma or equivalent preferred. Active Illinois Driver's License required. Must possess the ability to communicate effectively, orally and in writing.

PHYSICAL DEMANDS

The physical demands are representative of those that must be met by an employee to successfully perform the essential functions of this job. Must be able to push, pull move and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move and/or carry such weight a minimum distance of 50 feet. May be necessary to assist in the evacuation of residents during emergency situations. The noise level in the work environment is usually moderate.
