(Starting pay will be $20 per hour) This role will include, but not limited to: Manage and schedule all medical related appointments for all residents in Brain Care facilities. Provide transportation to and from medical appointments using company vehicles (must be an approved driver and have a valid driver license) Effectively communicate with providers and staff before, during, and after appointments as needed Monitor, maintain, and record all provider's notes, referrals, and recommendations using company's software and equipment or State compliance within an ALF Discuss with participants about their medical needs/follow up Connect participants with resources that align with their progression plans to enhance their rehabilitation Assist participants in exam room and/or within the houses Communicate medical information to the leadership team to help make decisions of correct course of action for appropriate client care Attend all management meetings and required staff meetings as assigned As member of the leadership team, you will be included in an on-call rotation schedule. Aid with transportation with outings, events, and other non-medical related appointments with participants when needed Will assist floor staff and shift coverage as needed (up-to-date certifications will be required) Able to provide transportation for emergency appointments as needed Must possess excellent organizational skills, effective time management, and have a positive professional demeanor
