Job Description
Under the direction of the RN, ensures high quality medical care and services by coordinating, scheduling, communicating and documenting medical appointments and medical issues for consumers. Includes transportation to and from appointments. Minimum Education & Experience High School diploma. Experience in the field of intellectual and developmental disabilities preferred. Licensure/Certification Certified AMAP and familiarity with medical terminology. Valid NYS driver's license. Must have access to timely and reliable transportation to transport self to multiple sites. The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, internal equity, and location.
