Position Summary:
The Medical Transport Driver & Activities Assistant serves a dual role in supporting the physical, emotional, social, and spiritual well-being of elders residing in Rochester Presbyterian Home memory care communities. This position safely operates company vehicles to transport elders to medical appointments, community outings, and other scheduled destinations while providing compassionate accompaniment and support throughout the transportation experience. When not engaged in transportation duties, the Medical Transport Driver & Activities assistant functions as an active member of the Engagement Team, assisting with recreational programming, therapeutic activities, resident companionship, life enrichment initiatives, community events, and individualized engagement opportunities. This position helps create meaningful experiences that promote purpose, dignity, independence, and quality of life for elders living in our memory care communities. RPH serves elders with memory impairment and is licensed and certified by the NYS DOH to provide
ALR, SNALR, EALR
levels of care. The Medical Transport Driver & Activities Assistant serves as a member of the interdisciplinary team and is (or will be) specifically trained in the supportive care of people with memory impairment, dementia, and other age-related diagnoses to effectively implement elders' individualized wellness care plans.
Essential Job Functions:
Resident Transportation and Safety Safely operate RPH-owned vehicles to transport elders to medical appointments, therapy visits, community outings, and other approved destinations. Perform routine vehicle inspections before and after use and promptly report maintenance, safety, or operational concerns. Accompany elders during appointments as directed, providing reassurance, support, advocacy, and supervision. Observe elders throughout transportation and report changes in condition, behavior, or mood. Ensure elders are safely assisted into and out of vehicles using appropriate safety practices and mobility assistance techniques. Secure wheelchairs, walkers, and other mobility devices as required during transport. Resident Engagement and Well-being When not performing transportation responsibilities, actively participate in the planning, preparation, implementation, and evaluation of engagement and activity programs. Facilitate group and one-on-one engagement opportunities that promote socialization, cognitive stimulation, physical activity, sensory experiences, creativity, and purpose. Provide companionship and emotional support to elders adjusting to the community, experiencing anxiety, returning from hospitalization, or requiring additional individualized attention. Implement assigned activities and engagement events. Dementia Care and Resident Support Act in a professional manner at all times and create a warm atmosphere of positivity, calmness, respect, and hospitality. Employ a person-centered approach that enables elders to function as independently as possible Validate, reassure, redirect, and support elders experiencing confusion, anxiety, distress, or other dementia-related behaviors. Redirect unsafe wandering and behaviors into meaningful and productive activity opportunities. Support elders in daily routines and recreational opportunities while preserving dignity, self-esteem, and personal choice. Work collaboratively with nursing, engagement staff, leadership, families, and other team members to ensure seamless resident support. Report all resident incidents and accidents during the shift when they occur. Report all grievances or concerns. Clean up after activities and engagement. Ensure that work areas are kept in a clean, safe, and sanitary state. Follow all RPH policies, procedures, infection prevention practices, and safety requirements.

Requirements:
Must be 18+ years of age. High school diploma or GED equivalent. Certificate of training from a DOH approved training program is a plus. Must possess valid and clean Driver License with an acceptable driving record and remain insurable under the organization's automobile insurance program. Ability to operate organizational vehicles in varying weather and traffic conditions. Previous experience in long-term care, assisted living community or ALR homecare setting working with older adults is a plus. Previous experience transporting older adults, individuals with dementia, or vulnerable populations preferred. A positive attitude, encouraging and motivating elders and staff to find joy, happiness and contentment. Ability to communicate in English (reading, speaking, and understanding). Possess interpersonal and communication skills to communicate tactfully with co-workers, elders, family members, visitors, government regulators and the public. Able to work independently and collaboratively as part of a team. Possess ability to make mature, independent decisions in a timely manner. Be a quick learner, empathetic, compassionate, and with ability to preserve the dignity and self-image of elders. Able to act professionally - arrive on time for your shifts, communicate variances as needed. Able to work flexible hours, including evenings and weekends. Willingness to attend and participate in on-the-job training. Ability to maintain confidential information and treat all elders and staff with respect.
