Responsibilities:
Provide safe, reliable transportation for PACE participants to appointments and program activities while delivering exceptional customer service. Assist participants with mobility needs, including wheelchair transport, ambulation, transfers, and home access. Monitor participant well-being, communicate changes in health or home conditions, respond appropriately to emergencies, and help create a welcoming, supportive environment. Follow transportation schedules, safety procedures, and all traffic laws while working independently and collaboratively with the care team. Maintain vehicles in a safe, clean, and ready-to-operate condition through daily inspections, fueling, and routine safety checks. Monitor vehicle performance, report maintenance needs, and support reliable transportation services across the PACE program. Participate in an after-hours on-call rotation as needed.
Qualifications:
Education:
High School graduate/GED required, or equivalent work experience.
Experience:
One (1) year of experience working with the frail or elderly population preferred. At least six (6) months experience transporting individuals with special mobility needs strongly preferred. License(s)/Certification(s): Must possess and maintain current Basic Life Support (BCLS) within 30 days of hire. Valid driver's license (Chauffer's Endorsement for IA license if determined necessary by Market), and good driving record required.
