Job Description

Essential Function Provide safe and reliable transportation to individuals with a range of disabilities in a wheelchair accessible van in order to gain access to community, events, supports, and services. Ensures all documentation is kept in compliance and submitted in a timely manner. Principal Activities Provides transportation for Medicaid recipients to gain access to community, events, supports and services. This position is responsible for working within WNY Integrated Care social care technology and transportation platforms. Maintain a valid driver's license Maintaining a clean and safe driving environment. Abide by all road and safety rules and regulations. Establish effective communication channels with consumers and their providers. Complete all required trainings. Able to work independently and be a team player. Be accountable for all work-related job duties Contributes to team effort by accomplishing related results as needed or requested by supervisor and/or Executive Directors. Job Qualifications High School Diploma or GED Desire and compassion to work with diverse populations .
  • Valid driver's license with clean driving record is a MUST Current auto insurance Experience in a Human Service environment. Communicates well verbally and in writing. Proficient in Microsoft Office. Maintains a clean and safe working environment. Effectively manages multiple tasks and priorities Understands and performs job results in a non-judgmental manne r.
  • Ability to interpret policies, procedures, and other written materials .
