The Van Driver / Resident Transportation Coordinator is responsible for safely transporting residents to and from medical appointments, therapy appointments, and other approved destinations. This position is also responsible for coordinating and scheduling resident transportation needs, maintaining an organized transportation calendar, communicating appointment details with residents, families, facility staff, and healthcare providers, and ensuring residents receive timely and dependable transportation services. The ideal candidate is dependable, compassionate, organized, safety-conscious, and comfortable working with elderly residents and individuals who may have mobility limitations or special transportation needs. Essential Duties and ResponsibilitiesResident Transportation Safely transport residents to and from medical appointments, therapy, dialysis, dental appointments, hospital visits, personal appointments, and approved community activities. Assist residents with entering and exiting the vehicle as needed while maintaining dignity, privacy, and safety. Secure wheelchairs, walkers, and other mobility equipment properly before transportation. Operate facility vehicles in a safe, courteous, and professional manner. Follow all applicable traffic laws, facility policies, and transportation safety procedures. Perform pre-trip and post-trip vehicle safety checks. Maintain the cleanliness and general condition of the facility vehicle. Report vehicle maintenance, mechanical issues, accidents, or safety concerns immediately. Scheduling and Transportation Coordination Schedule and coordinate resident transportation based on appointments, facility needs, and vehicle availability. Maintain an accurate daily and weekly transportation schedule. Confirm appointments and transportation requirements with medical offices and other destinations when necessary. Coordinate transportation schedules with nursing, therapy, social services, residents, and families. Ensure adequate travel time is allowed between appointments. Monitor changes, cancellations, and rescheduled appointments and update the transportation calendar accordingly. Communicate transportation delays or schedule changes promptly to appropriate staff and residents. Help prioritize transportation requests based on medical necessity, appointment times, and facility needs. Resident Care and Customer Service Treat every resident with dignity, respect, patience, and compassion. Provide courteous and professional service to residents, families, healthcare providers, and community members. Recognize and communicate changes in a resident's condition or transportation needs to appropriate facility personnel. Follow resident-specific safety, mobility, and assistance instructions provided by the facility. Maintain resident confidentiality and comply with HIPAA and facility privacy requirements. Assist residents with belongings, mobility devices, and personal items as appropriate. Documentation and Communication Document transportation-related incidents, delays, refusals, or concerns according to facility policy. Communicate effectively with nursing and other departments regarding resident transportation needs. Verify that residents are transported to the correct destination and returned to the facility safely. Maintain appropriate records of vehicle inspections, fuel usage, mileage, and maintenance. Qualifications High school diploma or equivalent preferred. Valid driver's license with a clean or acceptable driving record. Previous experience transporting seniors, residents, patients, or individuals with disabilities preferred. Experience working in a long-term care, assisted living, skilled nursing, healthcare, or similar environment preferred. Ability to safely operate a wheelchair-accessible vehicle. Ability to communicate effectively with residents, families, healthcare professionals, and facility staff. Strong organizational and scheduling skills. Ability to remain calm, patient, and professional in stressful situations. Physical Requirements Ability to sit and drive for extended periods. Ability to assist residents with entering and exiting the vehicle. Ability to safely secure wheelchairs and mobility equipment. Ability to assist with walkers and other mobility devices. Ability to occasionally lift, push, pull, or maneuver equipment and items consistent with facility requirements. Ability to respond appropriately to resident safety concerns and emergencies.
