ASAP Job responsibilities Communicate clinically relevant updates to providers to support timely decision-making. Support care coordination activities across providers to promote continuity of care. Document care delivered, patient response, and communications in the medical record. Educate patients on treatment plans, follow-up instructions, and self-management strategies.
