Schedule/Hours:

40 hours per week See 5-6 patients per day

Key Qualifications:

Valid license/certification as a Physical Therapy Assistant Recent clinical references: one within past 12 months, one within past 3 years Prior home health experience preferred; new grads considered in exceptional cases Must not have held direct employment with CommonSpirit-affiliated facilities in the past year (PRN with outside agencies may be accepted) Comfortable treating adult and geriatric populations with varied diagnoses

Role Responsibilities:

Deliver high-quality rehab therapy in patients' homes across greater Sacramento County Manage a caseload reflecting a range of therapeutic needs and functional levels Document care accurately and in compliance with facility protocols Collaborate during bi-monthly in-person or remote meetings for timesheet and mileage approval Meet productivity expectations of 5-6 daily visits
