In this role, you will be responsible for providing excellent patient care and support. You will play a crucial role in ensuring the safety and well-being of our patients by having an in-depth knowledge of treatment protocols, measuring wounds, documentation, etc. Our Wound Care Nurse will work closely with a team of healthcare professionals, including physicians, technicians, and other nurses, to deliver high-quality care. Duties and Responsibilities:

Provide direct patient care for individuals with wounds, including assessment, treatment, and ongoing monitoring. Collaborate with the interdisciplinary healthcare team to develop and implement individualized wound care plans. Perform wound dressings, debridement, and other wound care procedures. Educate patients and their families on proper wound care techniques and prevention strategies. Document patient assessments, interventions, and outcomes accurately and timely. Maintains accurate treatment schedule, timely records of treatments administered, progress notes, and evaluations for skilled residents Reviews all skin assessment of new residents. Conduct weekly meetings with Wound Committee Completes rounds with Wound Doctor every week and updates PCC Promote and respect the dignity and well-being of each resident and their family.

Required skills and qualifications:

IL State Nursing License 1+ year of experience as a Wound Care Nurse Graduate from accredited school of nursing, BSN preferred Two years of long-term care experience. Current nursing license for the state of IL. CPR certified Must enjoy working with the senior population Wound Care Coordinator | Wound Care Nurse |
