Job Description

Position Summary As a Wound Care / Ostomy Nurse, you will assess, treat, and support patients with complex wounds, ostomies, and continence conditions. You'll collaborate with interdisciplinary care teams to create and implement personalized treatment plans that promote healing, prevent infection, and improve long-term outcomes. What You'll Do Provide expert assessment and management of acute and chronic woundsEducate patients and families on wound and ostomy care for successful at-home managementDeliver specialized care for skin conditions and infections under physician directionDetermine appropriate levels of care (acute, long-term, outpatient)Develop and implement prevention programs for at-risk patientsLead patient and staff education on wound prevention and ostomy careCoordinate discharge planning with providers and case managementApply advanced knowledge of GI, GU, and integumentary systemsUtilize current best practices and standards in wound managementStage wounds accurately and adjust care plans based on patient responseServe as a clinical resource for physicians and staffStay current with evolving treatments, products, and research in wound careEnsure accurate, timely documentation in compliance with standardsSafely operate clinical equipment and support patient care coordination Qualifications Active RN or LPN license in Colorado (required)WOCN Certification within 6 months of hirePrevious experience in wound/ostomy careBLS certification requiredBachelor's degree preferred Why Join Delta Health?
