Job Description

Position Summary Delta Health is seeking a compassionate and dedicated Certified Occupational Therapy Assistant (COTA) to provide occupational therapy services to patients in their place of residence on an intermittent basis. Services are delivered in accordance with physician orders and the established plan of care under the direction and supervision of the Occupational Therapist (OT) and Director of Rehabilitation. The ideal candidate is patient-focused, detail-oriented, and committed to helping individuals achieve their highest level of independence and functionality. Key Responsibilities Administer occupational therapy treatments and interventions as directed by the Occupational Therapist and physician orders. Follow established rehabilitation policies, procedures, and care plans. Assist patients in improving or minimizing residual physical disabilities. Support patients in achieving their optimum functional and productive level within their capabilities. Collaborate with healthcare team members in patient care planning and coordination. Perform skilled procedures according to the OT-established plan of care. Communicate with the Occupational Therapist regarding patient progress and treatment modifications. Educate patients and family members on home exercise programs and fine motor movement activities. Prepare and maintain accurate medical records and timely documentation for each patient visit in accordance with agency policy. Assist in the preparation of clinical notes and progress reports. Participate in patient and family education initiatives. Attend patient care conferences and interdisciplinary meetings as scheduled. Participate in in-service education, staff training, and performance improvement activities. Qualifications & Requirements Graduate of an accredited Occupational Therapy Assistant program. Certified Occupational Therapy Assistant credential in accordance with the American Occupational Therapy Association (AOTA) requirements. Current state certification/licensure in the state(s) of practice. Minimum of two (2) years of experience preferred. Strong communication, interpersonal, and organizational skills. Ability to work independently while maintaining collaboration with the rehabilitation team. Physical Requirements Candidates must be able to: Participate in physical activity for extended periods. Stand and move frequently throughout the workday. Perform moderate lifting as needed. Regularly bend, lift, and stand during patient care activities. Why Join Delta Health?
