Job Description

Avanti Home Health Care is currently seeking a qualified candidate for an office admin assistant/Intake Coordinator seasonal/temporary- may lead to a permanent part time or full time - position in Novi. The Intake Coordinator is responsible for the overall coordination of Agency referrals, tracking schedules of clinicians and other office duties. The Intake Coordinator is a resource person for referral sources, and works closely with clinicians of the team to ensure adequate coordination and transition occurs to home health services. Hours are 10am-4pm
Responsibilities:
This individual is responsible for taking referrals and data entry Verifies medical eligibility for all patients prior the initiation of services Tracks and follows up on all MD orders in a timely manner Handles request for services either by phone or fax Assists in the scheduling of Home Health clinicians in coordination with the Director of Nursing Maintains a high visibility and availability in the office Reports employee/patient issues to the Director of Nursing. Orders medical and office supplies and maintains inventory Performs other related office duties
Qualifications:
Must be organized Detail oriented Computer skills in Microsoft Office (Excel, Word) and Microsoft Outlook Experience with intake process and procedures preferred Effective written and verbal communication Clean background and drug screen
