Job Description
HH Care Manager Supervisor Job Summary Step into a leadership role as a Care Coordination Supervisor supporting individuals with complex medical, mental health, and social needs through the Medicaid Health Home Program. In this full-time position, you'll oversee a team of care managers, improve service quality, and help drive client-centered outcomes. Ideal candidates have experience in care coordination, team supervision, and a strong commitment to improving community health. Responsibilities as the
Children's Care Coordination Supervisor:
•
Team Leadership:
Supervise care coordinators through coaching, performance reviews, and regular one-on-one support to ensure service quality.
•
Program & Compliance Oversight:
Ensure timely documentation, chart accuracy, and adherence to HIPAA and Health Home program standards.
•
Client Support & Intervention:
Assist with complex cases, ER admissions, and critical incidents to maintain continuity of care and client safety.
•
Quality & Performance Monitoring:
Conduct chart audits, monitor data integrity, and support contract deliverables and performance outcomes.

Community & Administrative Engagement:
Lead outreach, support groups, and wellness events while preparing reports and facilitating team meetings. Qualifications for the
Care Manager Supervisor:
•
Education:
Bachelor's degree in Social Work, Psychology, Public Health, Human Services, or a related field is required.
•
Experience:
Minimum of 2 years of supervisory experience in a case management, care coordination, or Health Home program setting is required.
•
Technical Skills :
Proficient in Microsoft Office (Word, Excel, Outlook) and experienced with electronic health record (EHR) systems and care management databases. FMC and PSYCHES expertise. Knowledge of CANS system to enroll new clients.

Industry Knowledge:
Strong understanding of Health Home program regulations, social determinants of health, and services related to HIV/AIDS, mental health, housing, and substance use.

Skills & Attributes :
Excellent communication, leadership, and problem-solving abilities, with the ability to support team growth and promote client advocacy. The starting range for this full-time position is $55,000 - $56,500, depending upon experience. A Plus Early Intervention offers Paid Time Off.
