Job Description

Data Entry Specialist Freedom Psychiatry is hiring a detail-oriented Data Entry Specialist to support our psychiatric providers by documenting patient visits directly into our electronic health record (EHR) in real time. You'll sit in on provider visits, capture clinical information as it happens, and help keep our charts accurate and up to date — allowing our clinicians to stay focused on patient care instead of paperwork. What You'll Do Join provider visits (in-person or telehealth) and enter clinical documentation into the EHR in real time. Accurately capture patient history, assessments, diagnoses, medication changes, and treatment plans. Enter screening tool scores (e.g., PHQ-9, GAD-7) and other structured data into patient charts. Prepare same-day visit notes for provider review and sign-off. Maintain strict patient confidentiality in line with HIPAA. What You'll Need Fast, accurate typing and strong attention to detail. Familiarity with medical terminology; psychiatric/behavioral health terminology a plus. Comfort learning EHR systems and clinic documentation workflows. Professionalism and discretion when handling sensitive patient information. High school diploma or GED required; related degree or scribe experience preferred. Prior scribe, medical assistant, or clinical documentation experience strongly preferred. Good to Know This role involves documenting sensitive topics (e.g., mental health history, risk assessments) with accuracy and professionalism.
