Job Description
Position:
Intake Support Specialist (PT) The Intake Support Specialist provides administrative and follow-up support to the Centralized Intake team to ensure timely access to care, reduced no-show rates, and accurate referral tracking.Essential Duties:
Conduct appointment reminder calls, texts, and follow-up outreach. Conduct client re-engagement outreach for individuals who miss appointments or disengage from services. Conduct client satisfaction surveys and document outcomes. Follow up on missing intake documentation and required forms. Maintain waitlists, referral trackers, and outreach logs. Assist with referral follow-up and outcome reporting.Minimum Requirements:
High school diploma or equivalent. Strong communication, customer service, organizational, and multitasking skills. Computer proficiency and ability to learn and utilize electronic medical record systems.Preferred Qualifications :
Bilingual. Experience in behavioral health, substance use treatment, healthcare intake, care coordination, customer service, or call center operations.Position Status:
This is a part-time position. Monday•Thursday:
12pm•8pmFriday:
