This is a full-time, Monday-Friday in-office position offering opportunities for professional growth and advancement. As an Intake Specialist, you will play a vital role in our organization's mission to provide unparalleled service and valued companionship to our patients.
Responsibilities:
Contact and educate patients about Managed Long-Term Care (MLTC), Medicaid Advantage Plus (MAP) and Program for All-Inclusive Care for the Elderly (PACE) plans Process patient referrals, including demographic information, clinical documentation, and financial reimbursement details Verify eligibility and benefits for Medicare, Medicaid, Commercial, and Managed Care insurance plans through online portals and direct communication with insurance providers Coordinate with Managed Care representatives to confirm home care coverage eligibility and obtain initial evaluation authorizations Assist patient with re-enrollment, recertification, authorization, and vendor changes Accurately input information into internal systems to ensure timely reimbursement Maintain data entry of required documents and patient clinical records Report to the
Director of Business Development and Intake Qualifications:
Bilingual candidates: English and Spanish preferred Experience using HHA Exchange software required Experience working and navigating multiple software systems Proficient in customer service, communication, and follow-up skills Strong organizational skills with exceptional attention to detail Minimum of 1 years of home care experience required- Required Experience verifying Health Insurances Familiarity with Private Duty Nursing (PDN) and/or
NHTD/TBI
