Description Lifespan Family Services of PA, a 501 c 3, non-profit providing adoption and SWAN services throughout western and central Pennsylvania. LifeSpan has received significant community recognition for excellence in providing quality service.

General Summary:

HIRING BONUS

of $1,000 paid after 90 days of employment! The permanency specialist is primarily responsible for providing all Statewide Adoption and Permanency Network (SWAN) units of service as well as completing all facets of private adoptions.

Essential Duties and Responsibilities:

• Writing child profiles after training to do so
• Meeting with significant persons involved in the child's life as well as collaborating with county agencies and service providers
• Performing child preparation units of services after training
• Performing child-specific recruitment units of services
• Prepare families for the adoption process/Family Profile/Training
• Provide services related to placement and finalization
• Providing case management, support, and linking families to services throughout post-permanency units of services
• Completes SWAN benchmarks, documentation, training, etc.
• Other duties to include: participate in court proceedings, SWAN meetings, attend conferences, trainings, meet with the Director of Permanency, etc.

Education and /or experience:

• Bachelor's degree in an educational, medical, or human services field; OR
• An associate degree or relevant work equivalent experience, namely at least two years working full time with children Language skills:
