Job Description
We are recruiting for a Birth to Three Services Evaluation and Assessment Coordinator . About the organization Creative Interventions provides early childhood intervention services. We partner with family members and caregivers throughout the process, providing a comprehensive assessment to foster outcomes and objectives that are meaningful to the growth and development of children and family routines. We also provide support in community settings as needed, including assistance with transition to public school services or other community-based opportunities once a child turns 3 years old. Job Summary The Evaluation and Assessment Coordinator is an integral part of the Birth to Three team. The role is responsible for timely and accurate scheduling of Birth to Three eligibility evaluations and autism assessments and assisting with daily tasks to ensure that proper documentation and compliance measures are met. Duties include but are not limited to: Monitor referral lists. Contact families within established time standards. Send letters to families who have not responded to attempts to contact. Document all contact with family in the child's electronic record. Enters data such as eligibility data, insurance information and IFSPs into the state data system. Review referral concerns to determine appropriate evaluation team and to determine if additional interpretation support is needed. Coordinate Autism Assessments in accordance with agency policy. Coordinate Birth to Three Assessments with Birth to Three Coordinator. Create initial case files and maintain all information needed for the assessment and transfer process. Obtain family signatures when required. Maintain evaluation and assessment calendars and ensure appropriate options for across zones and specialties, reach out to providers for additional slots as needed based on referral rates or specific evaluation/assessment needs. Update for changes and cancellations. Ensure that evaluators have access to all necessary information for each assessment/evaluation prior to the appointment. Create and maintain folders including Welcome Packet, Exit Packet, Autism Assessment Packet, etc. Stay up to date with changes to agency or Birth to Three policy regarding autism assessments and adjusts practices as necessary. Track and submit autism assessment results in the Birth to Three data system. Send eligibility evaluations and assessment reports to families or outside providers as requested. Ensure receipt of diagnostic report from the family and/or follow up with appropriate medical providers to obtain the report if necessary. Speak and write in the family's primary language directly or through an interpreter to follow the script and task list appropriate to their referral program (comprehensive or autism specialty). Submits requests for interpretation and translation for clinical providers as needed. Assist with uploading or creating electronic versions of appropriate, up to date, forms in the electronic record or DocuSign (i.e. parent rights, written prior notice, appropriate consents, insurance forms, etc.) as instructed by the Birth to Three Coordinator or Birth to Three Program Director. Assist in auditing charts for compliance measures. Create files, scan, and organize documents. Cross train on duties to support the team. Strong organizational skills, with ability to effectively multi-task and prioritize. Successfully shift between phone communication responsibilities, and computer, documentation and data entry duties. Strong communication skills, communicate with compassion, with ability to keep conversations on track and productive. Outstanding documentation and follow through. Basic Excel (data entry), Docusign, MS Office. Prior experience working with Birth to Three programs preferred. Bilingual (Spanish) preferred. Associate's degree preferred. Valid Driver's license. Full time with benefits. Office hours M-F. Remote work option may be offered after fully trained.
