Job Description

Central Minnesota Council on Aging is seeking a full-time Minnesota Aging Pathways Resource Specialist, 40 hours per week within a Monday-Friday, 8:00 am - 4:30 pm schedule. In this role you will assist older adults and their families primarily through taking inbound calls on the Minnesota Aging Pathways call center, a free statewide resource service. As a Resource Specialist you will also be trained to provide expert Medicare and benefits counseling to help navigate the options available. Key Responsibilities Provide phone assistance to older adults and their families to evaluate their needs and connect them to community services and resources. Research service options and share information to offer comprehensive, objective support to clients regarding long-term care services, housing options, caregiver support, home services, and other public benefits. Provide advocacy or referral on behalf of a caller if required to obtain needed services. Provide information and education on the topic of Medicare and health insurance. Qualifications Bachelor's degree in social work, gerontology, nursing, or a related human service field. Preferably 2 or more years of professional experience in social services, such as long-term care, health care, social services, or lead agency settings. Location The position is based in St. Cloud, MN. Employees living within 45 miles of the office are eligible to follow a hybrid schedule with two days per week working on site. Employees residing within our 14-county service region (Benton, Cass, Chisago, Crow Wing, Isanti, Kanabec, Mille Lacs, Morrison, Pine, Sherburne, Stearns, Todd, Wadena, and Wright counties), but more than 45 miles from the office are eligible for a fully remote work arrangement, based on organizational needs.
