Integrated Care Management Program Shift:
Part-time - 0830-1700 / 40 Hours per Pay The salary range begins at $17.00, reaches a midpoint of $19.35, and tops out at $23.21. The Case Management Assistant is responsible for accurate and timely administrative assistance and technical support to the Director and Managers of Integrated Care Management and other designated departments. This individual performs a wide range of clerical and technical tasks necessary to facilitate the smooth functioning of the department. Other responsibilities include project requests, important data collection and tracking, meeting preparation, recording of minutes, follow-up task, helping ensure positive patient experience, and effective coordination of care. Other assignments will vary based on department needs.

High School completion / GED Associate degree preferred Licensure/Certification/Registration §

1 - 3 years of job-related experience Prior job title or occupational experience:
MA, HUC, PCT, STNA
Prior specific functional responsibilities: Experience with Epic Preferred experience: Healthcare, Medical Terminology Other experience requirements: N/A Knowledge/Skills § Communication skills (oral and written) are imperative as they interact with physicians, employees, and patients. In additional there is daily contact with customers, employees, visitors, vendors, physicians and with patients/families § Working knowledge of Microsoft Office suite programs with excellent verbal, written communication and customer service skills. § Good organizational skills and ability to multi-tasks required. § Ability to assist staff with technical troubleshooting of software and hardware issues § Must be able to work well under pressure and maintain professional demeanor under adverse conditions § Skilled in self-directed work
