JOB SUMMARY
The Service Coordinator uses discretion and good judgment to interpret and articulate Optimae LifeServices' Mission, Vision, Core Values, and polices to empower customers and staff in accordance with customer desires and applicable standards. Fosters teamwork by encouraging cohesion, joint efforts, and interdependence. Reports directly to the applicable Regional Administrator, and serves as a member of the program's leadership team. Supervises Support Staff, Team Leaders, and other staff providing services to customers.
EDUCATION & WORK EXPERIENCE
Must be at least 18 years old. A high school diploma or GED is required. A bachelor's degree from an accredited college or university is preferred. With a bachelor's degree from an accredited college or university, a minimum of one (1) year of experience in the delivery, planning, coordination, or administration of human services is required. With an associate degree in a human services field, a minimum of two (2) years of experience in the delivery, planning, coordination, or administration of human services is required. With a high school diploma or GED, a minimum of three (3) years of experience in the delivery, planning, coordination, or administration of human services is required. Basic computer skills are required.
DUTIES AND RESPONSIBILITIES
May coordinate and approve all admission and funding procedures including gathering all intake information, service planning, and completing assessment of all needed Optimae LifeServices' Services. Coordinate and lead the service delivery and service planning process to ensure the provision of services based on customer desires and goals. Coordinate and lead staff coaching using pro-active problem-solving, team-based methods for customer-driven services, and monthly staff plans of development. Coordinate maintenance of comprehensive customer records in compliance with DIA, DHS, CARF, and other applicable standards. Coordinate services and procedures to provide for the health and safety of the customer. Maintain human and civil rights in a manner consistent with Optimae LifeServices' mission and core values. Coordinate all transfers and discharges with customers, including discharge planning, future service planning and review. Work in coordination with administration to market Optimae LifeServices' services, collect data and maintain a management information system, design, measure and implement needed improvements, and recognize accomplishments. Provide direct service when it is necessary to meet the customers health and safety needs. Provide on-call support by carrying a cell phone and/or pager when service needs dictate. Respond to calls in a timely matter and be available to provide service to customers when needed. Perform other tasks as needed. Work as a team member to achieve established financial objectives. Interview, hire, discipline and terminate employees as needed. Community Engagement Initiative in Iowa County and Surrounding Areas To effectively raise awareness about Optimae and foster strong community connections, we will implement a targeted engagement strategy in Iowa County and neighboring regions. This initiative will involve collaboration with local schools, chambers of commerce, community organizations, and other key stakeholders to ensure our message reaches a broad and diverse audience.

Host and participate in local events, health fairs, and workshops to engage directly with residents, answer questions, and build trust within the community.
