2989950416-2 Apply for this job now Job Description Sevita is seeking a Periodic Home Site Supervisor to lead person-centered home and community-based services. In this role, you'll oversee day-to-day operations for individuals receiving periodic in-home supports, ensuring safe, high-quality care aligned with individualized service plans. You will supervise and coach direct care staff, coordinate schedules, and maintain detailed documentation and compliance with healthcare and safety regulations. Collaborating closely with families, case managers, and clinical partners, you'll monitor progress, address concerns, and respond to incidents. Responsibilities Supervise day-to-day operations of periodic home support services for assigned individuals Create, implement, and monitor individualized service plans and goals Coach, schedule, and support direct care staff to ensure high-quality person-centered care Ensure compliance with healthcare, safety, and documentation standards and agency policies Coordinate with families, case managers, and healthcare providers to support each person's needs Monitor home environments for safety, cleanliness, and regulatory compliance Respond to incidents, complete reports, and implement corrective actions as needed Track and report outcomes, progress notes, and service utilization data Support staff training, performance feedback, and ongoing professional development Participate in on-call or rotating coverage as needed to support program needs Required Skills Knowledge of home- and community-based services (HCBS) and person-centered practices Care planning and documentation in health or human services settings Staff supervision, scheduling, and performance coaching Basic clinical observation and reporting skills Incident reporting and risk management Familiarity with healthcare and safety regulations for in-home services Electronic health record or case management software proficiency Time management and caseload coordination Communication and family/stakeholder coordination Basic Microsoft Office or similar productivity tools
