Provide comprehensive primary healthcare services to patients in their homes and in assisted living facilities. Conduct thorough patient assessments, develop treatment plans, and manage acute and chronic conditions. Order and interpret diagnostic tests. Prescribe medications and manage patient prescriptions. Provide patient education and counseling on health maintenance and disease prevention. Collaborate with other healthcare professionals to ensure coordinated and comprehensive care. Maintain accurate and timely electronic health records. Adhere strictly to all clinical protocols, compliance regulations, and best practices to ensure the highest quality of care. Travel to patient locations as required.

Qualifications:

Current, unrestricted licensure as a Nurse Practitioner or Physician Assistant in the state of Michigan. Board Certification in a relevant area (if applicable). Experience in primary healthcare is required. Experience in home health or geriatric care is a strong asset. Must possess a valid driver's license and reliable transportation. Demonstrated commitment to providing high-quality, patient-centered care. Excellent clinical assessment and diagnostic skills. Strong communication and interpersonal skills, with the ability to build rapport with patients and families. Ability to work autonomously while also collaborating effectively with a remote team. Exceptional organizational and time management abilities. Dedication to maintaining strict compliance with all healthcare regulations and internal policies.
