Haven Health, the leading Home Health, Hospice, and Palliative Care company in Southern California, is currently in need of an MSW (Masters of Social Work) to join our Social Services team. Individuals should be committed to serving patients and their families with Compassion, Dignity, and Grace. New Grads welcome! Will train the right candidate. We also believe education is an important part in our employees' growth. Clinical Supervision is available for MSWs working towards licensure. Positions Available:
Full Time; Hospice primarily

Responsibilities:
Assess the psychosocial functioning of Palliative and Home Health care patients and families in their home environment and develop an individualized plan of care Provide appropriate psychosocial interventions in accordance with the physician's orders and the policies and procedures of the Agency Educate patient/family on, and assist in, preparation of advance directives Provides information and referral to patients and families/caregivers regarding necessary community resources Maintains clinical records on all patients referred to social work prepares clinical and progress notes Function as an integral member of the interdisciplinary team

Qualifications:
Master of Social Work Degree New Grads welcome, we will train the candidate; clinical supervision is available Bilingual in Spanish and English People-oriented self-starter; notable written and verbal communication skills, and customer service skills California licensed and insured driver with reliable transportation Haven Health provides quality clinical care along with encouraging and empathetic support to help our patients reach their healthcare goals. We are centrally located in Signal Hill and serve patients from Los Angeles to Orange County. We are accredited by The Joint Commission and have over 15 years of experience in Hospice and Home Health care and 7 years of experience in Palliative Care.
