Job Description

Coordinating transportation Assessing benefit needs Refer internally to Centers Outpatient Mental Health Clinic, Primary Care Clinic, Pharmacy, Unhoused Team, Financial Coordinator Partner with local VA's and outreach programs This position is deemed safety-sensitive

Education & Experience:
