Job Description

Job Overview Position:
VCAP Program Staff Reports To:
Veterans Community Access Program (VCAP)
Program Manager Classification:
Non-Exempt Full time
Responsible For:
Management and oversight of the organization's services for its consumers. Includes aspects of management, administration, and service delivery.
Area:
This role requires travel within West Virginia. To support efficiency and reduce travel-related costs, the ideal candidate will reside in the western end of North Central West Virginia. All encouraged to apply, however.
Essential functions:
1. Ability to travel. 2. Ability to occasionally work long hours and/or weekends.
Duties:
1. Service Provision a. Receive VCAP referrals, including Veteran assessment information and an assigned annual allotment, from
LIVEABILITY
Program Manager. b. Within two business days of referral, the VCAP Advisor will contact the Veteran by telephone to schedule an in-home visit within seven business days whenever possible, to provide information about the VCAP, and to facilitate the Veteran in making an informed decision to participate or to decline to participate. c. Review the Veteran's support needs with the Veteran and assist him or her in the development of a Spending Plan. d. The VCAP Advisor shall provide information including, but not limited to:
  • An explanation of the Veterans Community Access Program, using the most current VCAP Handbook;
  • The person-centered planning process for developing a complete Spending Plan;
  • Veteran's rights and responsibilities in the program;
  • Resources, choices, and options;
  • Risks and responsibilities associated with VCAP and decision-making;
  • Program limitations or restrictions.
e. Report changes in the Veteran's condition and/or needs. f. Recognize and report critical events, abuse, or neglect. g. Request a review of decisions, reporting grievances, and dispute resolution. h. Communicate face-to-face and by phone with Veterans/Ars and, when requested by the Veteran, with others such as family members and/or caregivers. i. Remain in contact and be available to the Veteran as needed to accomplish the services of this Contract and shall make and document not less than one contact during each calendar month of the Veteran's enrollment in the Veterans Community Access Program (VCAP), and one face-to-face visit with the Veteran/AR quarterly. j.
Advise the Veteran how to:
  • Develop and document the Spending Plan to expend allotment to purchase services and/or goods that help the Veteran to continue living at home and avoid moving to a facility (facility means a nursing home, extended care facility, adult residential care home, foster family home, assisted living facility, or hospital)
  • Function as the employer of support workers in the home, including how to: o Develop or select a job description for an employee.
o Recruit, interview, check references, hire, manage, train and evaluate employees. o Consider appropriate issues when hiring a family member. o Set rates for employees' pay and arrange work schedules. o Develop a backup plan for when a scheduled employee fails to show up. o Develop a plan for emergency/disaster situations. o Dismiss and replace an employee, if necessary o Obtain the services of the FMS (what is FMS?). o Make decisions about the purchase of goods and services and use FMS to complete the purchases. o Assess the quality of goods and services received. o Seek additional information when needed to empower the Veteran in exercising self-direction. k. Provide support to all Veterans referred by LIVEABILITY or its designee. l. Transmit to appropriate parties, within a timely manner: the signed enrollment forms and packets, assessments, Spending Plan or other necessary documents. m. Assure each Spending Plan submitted can be carried out within the Veteran's assigned allotment and that it includes payments only for services and goods allowable and approved by Veterans Community Access Program (VCAP) policies. n. Assist the Veteran to submit monthly invoices with units of service itemized by Veteran within Ten calendar days after the last day of each month if needed. o. Transmit in writing to LIVEABILITY/ VA, within three business days, whenever a Veteran's residence status changes or a Veteran transition out of the program to include the reason for the change/transition. p. Develop and implement program policies and procedures congruent with the philosophy of Veterans Community Access Program and consistent with VCAP program policies. q. Exchange data electronically with
LIVEABILITY
in congruence with standards of the Health Insurance Portability and Accountability Act (HIPAA) r. Report any suspected or alleged abuse, neglect or exploitation to APS and

Program Manager. s. Other duties as assigned. 2. Documentation a. Maintain database of all VCAP applicants. b. Maintain quarterly reports of all consumers enrolled in the program with current status and submit to Program Manager. c. Renewal of yearly consumer paperwork to stay in compliance with audits. d. Provide supervisor with adequate documentation and time spent working with consumers, VA and FMS. e. Other duties as assigned. 3. Consumer Counseling a. Update consumers as to where they are with their budget. b. Assist with interpersonal troubles that may arise when consumers work with their employees. c. Problem solve any situations that may arise. d. Other duties as assigned. Qualifications 1. High School diploma 2. Experience working with people with disabilities
Required Skills:
1. Demonstrated ability to communicate effectively, orally and in writing, with or without reasonable accommodation. 2. Familiarity in using computers for work processing, data collection and communication. 3. Demonstrated ability to conduct research and analyses of pertinent legal authority.
