VP of Business Development Position Overview Phoenix Ortho Injury Spine & Extremity is seeking a motivated, compassionate, and highly organized Care Coordinator to serve as a primary advocate and point of contact for patients throughout their course of medical care. The Care Coordinator plays an essential role in helping patients navigate their treatment from their initial appointment through completion of care. This individual works closely with patients, providers, outside medical offices, referral partners, and internal team members to ensure that treatment plans are understood, appointments and referrals are coordinated, and patients remain engaged and on track with their recommended care. This is a highly communicative role that requires someone who is comfortable building relationships, spending significant time on the phone, proactively following up, and taking ownership of a patient's care coordination needs. Key Responsibilities Serve as a primary point of contact and advocate for assigned patients throughout their treatment with Phoenix Ortho Injury. Develop strong relationships with patients and provide consistent communication regarding appointments, referrals, treatment recommendations, and next steps. Help patients understand and navigate their treatment plans while ensuring they remain engaged and on track with their medical care. Monitor patient progress and identify missed appointments, scheduling delays, gaps in treatment, or other barriers that could interfere with continuity of care. Proactively contact patients when follow-up is needed and work to resolve barriers that may prevent them from completing recommended treatment. Coordinate outside medical referrals, including therapy, imaging, specialty care, and other services recommended by Phoenix providers. Follow up with outside medical offices to confirm appointments have been scheduled and completed and that necessary information is communicated appropriately. Maintain regular communication with referral partners and provide appropriate updates regarding patient scheduling and care coordination. Communicate closely with Phoenix providers, clinical staff, scheduling, intake, billing, and other internal departments to help create a seamless patient experience. Make a high volume of outbound telephone calls and follow-up calls as part of daily patient coordination responsibilities. Be comfortable making introductory or cold calls to outside medical offices, referral partners, or other organizations when necessary to coordinate patient care and develop needed resources. Document patient communication, scheduling activity, referral activity, and other relevant information accurately and promptly within the appropriate systems. Maintain an organized caseload and prioritize patients requiring immediate follow-up or additional assistance. Recognize when a patient requires additional attention and appropriately escalate concerns to leadership or the clinical team. Maintain patient confidentiality and follow all applicable HIPAA requirements and company policies. Provide a high level of professionalism, urgency, empathy, and customer service in every patient and partner interaction. What We're Looking For The ideal Care Coordinator is someone who genuinely enjoys helping people and takes pride in being an advocate for patients. This position requires a person who does not simply wait for problems to be brought to their attention—they proactively identify what needs to happen next and work to keep the patient's care moving forward.
Successful candidates should be:
Highly organized and able to manage multiple patients, referrals, appointments, and follow-up responsibilities simultaneously. Comfortable spending a significant portion of the workday communicating by telephone. Confident making outbound calls, including calls to individuals or offices with whom they may not have an existing relationship. Professional, personable, and comfortable communicating with patients, medical professionals, referral partners, and internal leadership. Persistent and proactive with follow-up while remaining compassionate and patient-focused. Detail-oriented with strong documentation and time-management skills. Able to work independently while also collaborating closely with a larger care coordination and clinical team. Comfortable working in a fast-paced environment where priorities can change throughout the day. Passionate about helping patients receive timely, appropriate, and coordinated medical care. Preferred Experience & Qualifications Previous experience in healthcare, medical scheduling, patient coordination, case management, medical administration, or a related field is strongly preferred. Familiarity with orthopedic and interventional care is highly beneficial. Working knowledge of common medical terminology, imaging, therapy, procedures, and specialty referrals is preferred. Experience managing a patient caseload or coordinating care between multiple medical providers is a plus. Strong computer skills and the ability to learn electronic medical record systems, communication platforms, and other practice-management technology. Spanish-speaking/bilingual candidates are strongly preferred but not required. Work Structure This position begins as a fully in-person role for the first six months of employment . This period is designed to provide hands-on training, allow the Care Coordinator to develop a strong understanding of Phoenix workflows and services, and build relationships with the clinical and operational teams. After successfully completing six months and demonstrating the ability to independently manage the responsibilities of the position, the Care Coordinator becomes eligible for our hybrid schedule of: 2 days per week in office 3 days per week working remotely Hybrid eligibility is dependent upon successful performance and the employee's ability to consistently meet the expectations of the Care Coordinator role. The Impact of This Role Our Care Coordinators are more than schedulers. They are an important part of the patient's experience with Phoenix Ortho Injury. Patients may be navigating multiple appointments, imaging, therapy, procedures, outside referrals, and unfamiliar medical terminology at the same time. The Care Coordinator helps bring those pieces together.
The goal is simple:
make sure patients know what comes next, help remove obstacles to receiving care, and ensure no patient gets lost in the process. Pay:
$50,000.00 - $60,000.00 per year
