We are a progressive organization, dedicated to helping people - including our staff - be successful! LifeSpring Health System is seeking Care Coordinators for the Zero Suicide program. Care Coordinators will coordinate care for clients as they transition from crisis to outpatient services or transition from the emergency department, jail, or inpatient facility; ensure a warm hand-off; monitor those at high risk for concerning behavior; and serve as point of contact. Bachelor's degree in social work, psychology or a related field is required; experience in the behavioral health field and at-risk populations is highly desired. The successful candidate will be willing to travel some, and possess excellent oral & written communication and organizational skills. We are looking for caring, compassionate and energetic people who enjoy helping others. An electronic medical record is used so computer literacy is also required.
