Care Coordinators will coordinate care for clients as they transition from crisis to outpatient services or transition from the emergency department, jail, or inpatient facility; ensure a warm hand-off; monitor those at high risk for concerning behavior; and serve as point of contact. Bachelor's degree in social work, psychology or a related field is required; experience in the behavioral health field and at-risk populations is highly desired. The successful candidate will be willing to travel some, and possess excellent oral & written communication and organizational skills. We are looking for caring, compassionate and energetic people who enjoy helping others. An electronic medical record is used so computer literacy is also required.
