Social Worker Case Manager Who We Are:

Job:

You will provide clinical social work interventions with patients, (neonates, pediatric, adolescents, adults, and geriatrics) and families that address personal and environmental issues to maximize emotional, social and physical wellbeing and the appropriate and efficient use of health care services across the continuum. You will provide a systematic approach to evaluation of patient care. Coordinate patient care through the continuum thereby facilitating the achievement of optimal quality outcomes in relation to clinical care and cost effectiveness.

What You'll Do:

Collaborates and participates with the primary nurse and the multidisciplinary team in the ongoing clinical social work assessment of patient's status/care management Assist in the development, implementation, and evaluation of integrated clinical pathways Monitors patient's progress to ensure care is appropriate and timely Develops discharge plan in collaboration with the interdisciplinary team Coordinate's referrals Utilization review

Who You Are:

Must demonstrate proficiency in assessing treatment and responses and adapting care to meet the needs of the patient population served; neonatal, pediatric, adolescent, adult, and geriatric Master of Science in Social Work required;

NH LICSW

preferred One (1) to three (3) years of experience in a health care system, preferably in a hospital setting Excellent clinical documentation, and successful usage of EMR systems preferred
