At Health Care for the Homeless, we believe healthcare is a human right. As a Transitions of Care Coordinator, you won't just manage referrals or track appointments, you'll help patients navigate complex healthcare systems and ensure they don't fall through the cracks during critical moments of care. Job Responsibilities:
Coordinate specialty referrals, diagnostic testing, and community partner services Obtain prior authorizations and communicate outcomes with providers Track referrals and follow up on incomplete appointments to prevent gaps in care Support smooth transitions following hospital discharge or specialty visits Arrange transportation and escorts to promote appointment adherence Assist patients in navigating healthcare and social service systems Monitor insurance portals and medication formularies and share updates with the team Collaborate with clinical and quality staff to identify patients needing outreach Support front-end intake and registration processes as needed Job Requirements High School Diploma or GED and 3-5 years of relevant healthcare experience Associate's or Bachelor's degree preferred Basic computer and data entry proficiency Compassionate, patient-centered approach to working with vulnerable populations Valid driver's license or reliable transportation for occasional travel between sites All qualified applicants will receive consideration for employment without regard to race, color, religion, sex or national origin, marital or familial status, age or mental or physical handicap Equal Opportunity Employer This employer is required to notify all applicants of their rights pursuant to federal employment laws.
