coordinates the delivery of home care services to Chinese-American patients in New York City and Long Island through a network of three Licensed Home Care Service Agencies (LHCSAs): ABI Healthcare, ATD Home Health Agency, and Life Improvement Homecare. ROLE SUMMARY
ABI, Life, and ATD are looking for a passionate, organized, and resourceful Care Coordinator to support our agency. This role is at the heart of our operations and serves as a primary point of contact for patients, families, and caregivers. You will manage and engage directly with a dedicated portfolio of patients and take ownership of keeping their home care services running smoothly. A major part of the role is finding and matching caregivers with patients, maintaining coverage when schedules change, and solving staffing challenges as they arise. This role is ideal for someone who enjoys helping people, building relationships, and finding solutions in a fast-paced environment.
WHAT YOU'LL DO
Coordinate & Staff Care:
Manage your patient caseload, identify and match caregivers based on patient needs and preferences, and find replacement coverage when caregivers call out or cases need to be re-staffed.
Manage Cases & Schedules:
Coordinate new case starts, authorizations, caregiver onboarding needs, schedules, and ongoing case changes while maintaining accurate information in our database and internal trackers.
Own Patient Relationships & Deliver Great Service:
Serve as a primary contact for patients, families, and caregivers, conduct regular check-ins, respond to concerns, and proactively support patient satisfaction and retention.
Solve Problems:
Navigate staffing shortages, schedule changes, caregiver availability, patient preferences, and other day-to-day challenges to maintain continuity of care.
Drive Collaboration:
Work with insurance plans and internal HR, Clinical, Compliance, and management teams to address patient needs, service changes, complaints, and other case-related issues.
Ensure Compliance:
Protect and maintain data integrity. Monitor caregiver punches and EVV performance, provide first-line support when needed, and escalate issues to Compliance specialists when appropriate.
WHAT YOU'LL BRING
Customer service, coordination, scheduling, staffing, or similar experience; home care experience is a plus. High school diploma or GED required. English + Mandarin and/or Cantonese required; additional Chinese dialects preferred. Strong problem-solving skills and persistence when facing difficult challenges. Excellent communication, relationship-building, organization, and multitasking skills. Comfort using computers and quickly learning new systems and tools. LOCATION
