Overview We are seeking a dedicated and detail-oriented MDS Assessor to join our healthcare team. In this vital role, you will be responsible for evaluating patient data, ensuring accurate completion of the Minimum Data Set (MDS) assessments, and supporting quality care initiatives across long-term care, skilled nursing facilities, and inpatient settings. Your expertise will help optimize patient care plans, ensure compliance with federal and state regulations, and facilitate seamless communication among interdisciplinary teams. This position offers an exciting opportunity to contribute to improving patient outcomes through precise documentation and clinical assessment. Responsibilities Completing accurate assessments, MDS & care plans as assigned Initiating care plans and supporting activities as assigned Creating and distributing monthly care plan calendars in a timely fashion Maintaining & updating all care plans and assessments as required Monitoring & auditing clinical records, ensuring accuracy & timeliness Protecting the confidentiality of Resident & Facility information at all times

REQUIREMENTS

MDS Experience Preferred Valid New York RN License Long Term Care Experience Required! Pay:

$50.00 - $58.00 per hour
