In this role, you will conduct outreach, complete intake assessments, coordinate care plans, schedule services, and collaborate with clinicians, pharmacies, and insurers. You'll track progress, resolve barriers to care, educate patients on resources, and document interactions in care management systems. Responsibilities Conduct telephonic and digital outreach to patients to support enrollment and engagement in care management programs. Complete intake assessments and gather clinical, social, and benefits information to inform care plans. Coordinate appointments, referrals, and services with providers, pharmacies, and health plans. Educate patients on benefits, community resources, and care options to reduce barriers to care. Document all interactions, assessments, and outcomes in care management and EHR systems with high accuracy. Monitor patient progress, follow up on care gaps, and escalate complex cases to clinicians or case managers. Collaborate with nurses, social workers, pharmacists, and insurers to ensure integrated, patient-centered care. Protect patient privacy and ensure full compliance with HIPAA and organizational policies.
