Principal Care Management Coordinator MD TruCare | Dallas-Fort Worth, Texas Full-Time Help Us Build a Better Way to Care for Patients MD TruCare is a growing, physician-led behavioral health organization serving patients across the Dallas-Fort Worth area. We provide comprehensive psychiatric care, psychotherapy, TMS, Spravato, and other advanced behavioral health services. We are looking for an exceptional Principal Care Management Coordinator who believes that great healthcare requires more than scheduling appointments—it requires ownership, follow-through, compassion, and relentless attention to the patient journey . This is not a passive administrative role. You will help identify patients who need additional care coordination, engage them between visits, communicate with clinicians and outside providers, track outstanding needs, and make sure important issues do not fall through the cracks. What You Will Own The Principal Care Management Coordinator will be responsible for helping manage a defined population of patients with significant behavioral health needs.

Key responsibilities include:

Identify and track patients who may qualify for Principal Care Management services based on established clinical and operational criteria. Contact patients, explain the care-management program, and support patient engagement and enrollment. Serve as a consistent point of contact for enrolled patients. Coordinate communication between patients, psychiatric providers, therapists, primary care physicians, pharmacies, specialists, and other members of the care team. Follow up on referrals, medications, laboratory testing, appointments, hospital or emergency department visits, and other outstanding care needs. Help patients overcome barriers that interfere with treatment. Escalate clinical concerns promptly to the appropriate licensed clinician. Maintain accurate and timely documentation of care-management activities. Track time and services accurately according to established Principal Care Management workflows. Maintain patient registries, tracking lists, and follow-up queues. Work closely with providers and leadership to identify patients who may benefit from higher levels of treatment or additional services. Participate in team huddles and performance reviews. Protect patient confidentiality and follow HIPAA and organizational policies at all times. Your Scorecard Success in this position will be measured—not guessed. Minimum Qualifications Associate or Bachelor's degree in Healthcare Management, Health Administration, Behavioral Health, Public Health, or a closely related healthcare field. At least 2-3 years of experience working in a medical, behavioral health, care management, or healthcare coordination setting preferred. Strong telephone and patient communication skills. Excellent organizational and follow-through abilities. Comfort working with electronic health records and multiple technology platforms. Strong written communication and documentation skills. Ability to manage a large number of patients and tasks without losing attention to detail. Ability to recognize when an issue needs to be escalated to a clinician. We Would Especially Like to Meet You If You Have Psychiatry or behavioral health experience. Care management, chronic care management, population health, or patient navigation experience. Experience communicating with primary care offices and specialists. Experience with eClinicalWorks or a similar EHR. Experience working with insurance authorization or healthcare documentation.
