Pomelo Care seeks a mission-driven Registered Dietitian to provide virtual nutrition care for individuals in midlife and caregivers in maternal, newborn, and infant health programs. In this contract role, you'll conduct telehealth assessments, design personalized, evidence-based nutrition plans, and collaborate closely with physicians, nurses, behavioral health, and care coordinators. You'll educate patients on chronic disease risk reduction, weight management, and family nutrition, document in the EMR, and use data and feedback to refine care pathways. This is a fully remote, flexible position within a fast-growing, impact-focused startup. Responsibilities Provide telehealth nutrition assessments for midlife patients and caregivers in maternal and infant health programs Develop individualized, evidence-based nutrition care plans aligned with clinical guidelines Educate patients and families on weight management, chronic disease prevention, and family nutrition Collaborate with physicians, nurses, behavioral health, and care coordinators to deliver team-based care Document visits, care plans, and outcomes accurately in the EMR and other clinical systems Use data and patient feedback to refine care pathways and improve outcomes at scale Participate in virtual team meetings, case reviews, and quality improvement initiatives Maintain licensure, certifications, and continuing education in nutrition and perinatal health as applicable Required Skills Clinical nutrition assessment Medical nutrition therapy (MNT) Telehealth platforms and workflows Electronic medical records (EMR) documentation Perinatal and maternal nutrition Chronic disease and weight management Care planning and patient education Motivational interviewing Data-driven quality improvement Interdisciplinary care collaboration
