Hospital / Long-Term Care / Skilled Nursing Position Overview We are seeking an experienced Registered Dietitian (RD/RDN) to provide clinical nutrition services in a hospital and long-term care/skilled nursing setting in New York City. The Registered Dietitian will assess patients and residents, develop individualized nutrition care plans, provide nutrition education, monitor nutritional needs, and collaborate with physicians and the interdisciplinary healthcare team. This position provides an opportunity to support patients and residents with a variety of nutritional needs and medical conditions. Key Responsibilities Complete timely nutrition screenings and comprehensive clinical nutrition assessments. Assess nutritional needs and develop individualized nutrition care plans. Provide follow-up nutrition care based on patient and resident needs. Review laboratory results, medical history, physical findings, and other relevant clinical information. Interview patients, residents, and family members regarding dietary habits and preferences. Provide nutrition and diet education to patients, residents, and families. Document assessments, interventions, education, and patient understanding in the medical record. Collaborate with physicians regarding appropriate diet modifications. Monitor and educate patients regarding food and medication interactions. Modify menus based on clinical and dietary requirements. Provide nutrition care across a variety of disease states and clinical conditions. Participate as a member of the interdisciplinary healthcare team. Support quality improvement and performance improvement initiatives. Required Qualifications Bachelor's degree in Dietetics, Foods and Nutrition, or a related field from an accredited college or university. Current Registered Dietitian (RD/RDN) credential through the Commission on Dietetic Registration (CDR) . New York State Dietitian-Nutritionist certification , as required for the applicable post-acute/long-term care setting. Minimum 1 year of relevant clinical experience . Experience with clinical nutrition assessment, nutrition care planning, diet modification, and clinical documentation. Ability to communicate effectively in a professional healthcare environment. Preferred Qualifications 2+ years of clinical dietitian experience. Master's degree in a relevant field. Hospital, long-term care, skilled nursing, or post-acute care experience. Experience with MDS 3.0 . Experience with electronic medical records such as PointClickCare or SigmaCare . Experience with electronic menu systems such as CBORD, NetMenu, or GeriMenu . Current AHA BLS certification , if required by the facility.
