The Patient Care Coordinator (PCC) plays a vital role in delivering on that mission by creating an exceptional, patient-centered experience from the very first interaction. As a key member of our care team and often the first point of contact, the PCC sets the tone for every patient's journey. This role combines patient engagement, scheduling coordination, and administrative support to ensure seamless, high-quality care across all clinic locations. The ideal candidate is highly organized, service-oriented, and thrives in a collaborative, fast-paced environment. Key Responsibilities
  • Patient Communication & Scheduling
  • Serve as the primary point of contact for incoming patient calls across multiple clinic locations
  • Schedule, confirm, and manage appointments for all providers across six clinic locations
  • Proactively contact patients to confirm appointments and ensure required paperwork is completed
  • Monitor and respond to online appointment requests in a timely manner Patient Experience & Front Desk Support
  • Create a warm, welcoming, and professional environment for all patients and visitors
  • Greet patients promptly and ensure providers are notified of patient arrival
  • Maintain a clean, organized, and inviting reception area
  • Deliver a "red carpet" level of service to every patient interaction Clinic Coordination & Operations
  • Manage and maintain accurate clinic schedules across all locations
  • Coordinate patient flow to optimize provider efficiency and patient satisfaction
  • Communicate effectively with clinical staff to support daily operations Administrative & Financial Responsibilities
  • Ensure completion and accuracy of patient intake forms and documentation
  • Identify and communicate insurance-related documentation requirements
  • Process patient payments and maintain accurate financial records
