Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods. Monitor patients' health by observing vital signs, tracking changes in condition, and reporting findings accurately to healthcare professionals. Support medication administration in accordance with care plans and HIPAA regulations to ensure safety and compliance. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking when necessary. Maintain a clean, safe environment by performing light housekeeping tasks such as cleaning patient areas and ensuring hygiene standards are met. Provide transportation assistance for appointments, errands, or community activities to promote social engagement and independence. Document care activities thoroughly in client records, including patient observations, progress notes, and updates on care plans. Qualifications Prior experience working in group homes, nursing homes, assisted living facilities, or home health environments is highly preferred. Knowledge of dementia care, Alzheimer's care, memory care, hospice care, and neurodevelopmental disorders enhances your ability to support diverse needs. Demonstrated ability to observe patients carefully and respond promptly to changes in condition; experience with patient monitoring is essential. Certification in First Aid and CPR is required; additional training in HIPAA compliance and manual handling techniques is advantageous. Strong communication skills to effectively interact with clients, families, and healthcare teams while maintaining confidentiality. Ability to perform heavy lifting safely and assist with patient transfers using appropriate equipment like Hoyer lifts or patient transfer methods.
