In this vital role, you will provide personalized, respectful, and comprehensive home healthcare assistance to individuals facing terminal illnesses. This paid position offers an opportunity to work closely with interdisciplinary teams, supporting patients' physical, emotional, and personal needs in the comfort of their homes. Responsibilities Deliver direct patient care by assisting with activities of daily living (ADLs), including bathing, grooming, dressing, and mobility support Monitor and record vital signs such as blood pressure, pulse, respiration, and temperature to track patient health status accurately Assist with medication administration following prescribed medical orders while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards Support patients with specialized needs such as catheter care, dementia or Alzheimer's care, memory care, and end-of-life comfort measures Utilize EMR (Electronic Medical Records) systems to document patient interactions, medical records, and care plans efficiently and accurately Employ patient transfer methods safely using equipment like Hoyer lifts or transfer belts to prevent injury for both patient and caregiver Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry and meal preparation in accordance with client care plans Requirements Proven experience working with individuals with disabilities, geriatrics, or in assisted living or nursing home settings Knowledge of medical terminology, medication administration protocols, and vital signs measurement Certification in CPR (Cardiopulmonary Resuscitation) and basic first aid required; BLS (Basic Life Support) certification preferred Familiarity with home health care assistance procedures including catheter care, dementia or Alzheimer's care, and hospice support services Ability to lift heavy objects safely and perform patient transfers using appropriate equipment like Hoyer lifts or transfer belts Strong communication skills for effective client interaction and collaboration with healthcare teams Understanding of home health agency operations, home & community care standards, and compliance with HIPAA regulations for maintaining patient confidentiality Experience working with individuals with developmental disabilities or in long-term care environments is highly desirable Valid driver's license and reliable transportation may be required for travel between patient homes Join us in providing compassionate hospice care that honors each patient's dignity. Pay:
$17.50 - $20.00 per hour Expected hours: 20.0 - 30.0 per week
