Job Description

We are looking for a compassionate and dependable Medical Social Worker (MSW) to join our growing team. In this role, you will work closely with patients, families, and our interdisciplinary care team to address the social, emotional, psychosocial, and financial needs that can arise during illness and hospice care. What You'll Do As a Medical Social Worker, you will: Provide social work services to patients and families receiving home health and hospice care Complete psychosocial assessments and social histories Assess patient and family needs, strengths, support systems, and available resources Assist patients and families with financial, social, and community resource needs Provide referrals to appropriate community agencies and resources with follow-up as needed Participate in developing individualized plans of care and discharge planning Collaborate with nurses, physicians, therapists, and other members of the interdisciplinary care team Participate in interdisciplinary group (IDG) meetings Provide education, support, and guidance to patients and their families Complete required clinical documentation, progress notes, assessments, and reassessments within established timeframes Maintain patient confidentiality and follow agency policies and procedures Participate in ongoing training, quality improvement, and team initiatives What We're Looking For Candidates should have: Master of Social Work (MSW) from a CSWE-accredited school of social work; OR Bachelor's degree in Social Work from a CSWE-accredited institution; OR Bachelor's degree in social work, psychology, sociology, or another related field with appropriate MSW supervision At least one year of social work experience in a healthcare setting Current Social Work license in good standing in the state of practice Valid driver's license Strong communication, organization, time-management, and problem-solving skills Ability to work independently while collaborating effectively with an interdisciplinary team Compassionate and professional approach to working with patients and families Why Heritage?
