Care Management / Agency Leadership Job Overview 24hr HomeCare Solutions is seeking compassionate, dependable, and dedicated PRN Private Duty Home Health Aides (HHAs) to provide personalized, high-quality care to clients in their homes throughout Lima, Ottawa, and surrounding Ohio communities. This position offers flexible scheduling and the opportunity to make a meaningful difference in the lives of individuals who need assistance remaining safe, comfortable, and independent at home. As a vital member of our care team, the HHA provides hands-on assistance with activities of daily living, personal care, mobility, companionship, household tasks, and other services outlined in the client's individualized plan of care. The ideal candidate is compassionate, energetic, detail-oriented, reliable, and committed to delivering exceptional client-centered care while maintaining professionalism, confidentiality, and compliance with agency requirements. Essential Duties & Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, hygiene, and personal care. Assist with mobility, ambulation, positioning, and transfers using appropriate techniques and approved equipment when trained and authorized. Provide assistance with gait belts, mechanical lifts, and other transfer equipment when required by the client's care plan and within the aide's training and permitted scope. Provide medication reminders and other medication-related assistance only as permitted by the client's plan of care, agency policy, and applicable scope of service. Observe clients for changes in condition, behavior, mobility, cognition, skin integrity, appetite, or overall well-being and promptly report concerns to the appropriate supervisor. Provide compassionate support to clients with dementia, Alzheimer's disease, developmental disabilities, cognitive impairments, or other special care needs. Provide supportive personal care to clients receiving hospice or end-of-life services while maintaining comfort, dignity, and respect. Prepare meals and assist with feeding when required. Perform light housekeeping, laundry, linen changes, and other tasks necessary to maintain a clean and safe client environment. Provide companionship, conversation, supervision, and meaningful engagement. Follow the client's individualized plan of care and assigned care sheet. Complete all required visit documentation accurately and timely. Clock in and out for every scheduled visit using the agency's Electronic Visit Verification (EVV) system. Maintain client confidentiality and comply with HIPAA and agency privacy requirements. Communicate professionally with clients, family members, agency staff, and other authorized members of the care team. Immediately report emergencies, incidents, injuries, suspected abuse or neglect, client concerns, missed visits, and significant changes in condition according to agency procedures. PRN Scheduling & Reliability Expectations This is an as-needed/PRN position. Hours are not guaranteed and are based on client needs, available cases, and caregiver availability. PRN caregivers may be contacted for: New client assignments Open shifts Call-off coverage Weekend assignments Evening assignments Short-notice coverage Additional hours as cases become available Once a caregiver accepts an assignment, the caregiver is expected to report to the client's home on time and complete the entire scheduled shift. Reliability and communication are essential. Caregivers must promptly communicate schedule concerns, emergencies, or circumstances that could affect an accepted assignment. Qualifications & Skills Previous experience in home care, home health, assisted living, memory care, long-term care, hospice, developmental disability services, or a related caregiving environment is preferred. HHA, STNA, CNA, or other relevant caregiving training or credential is preferred, where applicable. Current CPR/First Aid certification preferred or required based on assignment. Knowledge of safe transfer techniques, gait belts, and mechanical lifts preferred. Experience caring for seniors and individuals with dementia, Alzheimer's disease, disabilities, or complex personal care needs is highly valued. Strong client observation and documentation skills. Ability to recognize and promptly report changes in a client's condition or behavior. Ability to follow written care plans and instructions. Comfortable using a smartphone, EVV system, and electronic documentation systems. Excellent communication and interpersonal skills. Compassionate, patient, dependable, and professional. Ability to work independently in a client's home while remaining accountable to agency leadership. Ability to safely perform the physical functions necessary for assigned client care. Reliable transportation to assigned client locations. Valid driver's license and auto insurance when driving is required for an assignment. Ability to successfully complete required background checks, exclusion screenings, orientation, training, and agency onboarding requirements.
