Description We are seeking a compassionate, dependable, and patient-focused Home Health Aide (HHA) to provide personal care and supportive services to patients in their homes. This role is ideal for individuals who are passionate about helping others maintain their independence, dignity, and quality of life while receiving care in the comfort of their own homes. As a Home Health Aide, you will work closely with patients, families, nurses, and other healthcare professionals to deliver high-quality, personalized care while promoting safety, comfort, and well-being. Position Summary The Home Health Aide (HHA) provides personal care and supportive services to patients in accordance with the individualized care plan established by the Registered Nurse (RN) or other authorized practitioner. The HHA is responsible for observing and reporting changes in patient condition, reinforcing patient education, documenting services provided, and supporting patients with activities of daily living. Essential Duties
Maintain Pennsylvania registration and certification requirements for Home Health Aides.
Comply with all applicable federal, state, and local laws, regulations, and agency policies.
Provide personal care services in the patient's home as outlined in the care plan.
Follow and perform assigned tasks according to the established care plan.
Observe patient health conditions and promptly report changes or concerns to the supervisor.
Respond appropriately to emergencies in accordance with agency procedures and the patient care plan.
Reinforce patient and family education related to daily living activities, treatments, diet, safety, disease management, and overall wellness as directed by a Registered Nurse or authorized practitioner.
Maintain timely, accurate, and complete documentation of care provided.
Provide companionship and basic emotional support to patients.
Protect patient confidentiality, privacy, and rights at all times.
Represent the agency professionally when interacting with patients, families, referral sources, community partners, and the public.
Key Responsibilities
Patient Care
Assist patients with activities of daily living (ADLs), including personal hygiene, grooming, mobility, and other supportive care tasks.
Deliver care as directed by the patient's individualized care plan.
Promote patient comfort, independence, dignity, and safety.
Monitor and report physical, emotional, and environmental concerns observed during visits.
Communication & Documentation
Record services provided and maintain accurate patient care records according to agency requirements.
Inform supervisors promptly of changes in patient status or concerning observations.
Communicate effectively with nursing staff and other members of the healthcare team.
Assist in coordinating patient services and referrals when appropriate.
Professional Development & Teamwork
Participate in orientation, in-service training, continuing education, and staff development activities.
Attend scheduled meetings, training sessions, patient conferences, and educational programs.
Assist with quality improvement initiatives, including participation in the Agency's Quality Assurance and Performance Improvement (QAPI) program.
Contribute ideas and feedback to improve patient care and agency operations.
Establish personal professional goals and actively pursue growth opportunities.
Participate in community, educational, and professional activities as directed.
Qualifications
Required
High school diploma or GED.
Current licensure, certification, or registration as required by applicable federal, state, and local regulations.
Completion of an approved Home Health Aide training program meeting:
42 C.F.R. § 484.36 requirements
Conditions of Participation § 484.80 requirements
Pennsylvania § 601.6 requirements
Current Basic Life Support (BLS) certification.
Ability to pass Pennsylvania State and Federal Criminal Background Checks and Child Abuse Clearances.
Strong verbal and written English communication skills.
Positive attitude, professionalism, and dedication to patient care.
Ability to work independently while maintaining effective communication with supervisors and team members.
Preferred
Previous home health, personal care, caregiving, or healthcare experience.
Experience working with elderly, disabled, or chronically ill individuals.
Physical Requirements & Working Conditions
Perform patient care visits in patients' homes.
Lift, transfer, and assist patients weighing more than 50 pounds when appropriate.
Move household furniture and equipment as necessary for patient care.
Frequent standing, walking, sitting, bending, reaching, and stooping.
Travel by personal vehicle to patient homes throughout the service area.
Meet required documentation and scheduling deadlines.
Communicate effectively in person, by phone, and through electronic documentation systems.
