Your proactive approach and attentive care will promote independence, safety, and comfort for each patient you serve. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and observe patient conditions regularly, noting changes in health status and reporting findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Administer medications and treatments as prescribed, following all home healthcare regulations and safety protocols Perform vital signs assessments such as blood pressure, pulse, temperature, and respiration rates to track patient health trends Support patients with specialized needs such as Alzheimer's care, dementia care, hospice care, or end-of-life assistance with sensitivity and professionalism Assist with medical procedures including catheter care, wound dressing changes, and patient transfer methods using equipment like Hoyer lifts or other assistive devices Prepare nutritious meals tailored to dietary restrictions and assist with light housekeeping tasks such as cleaning, laundry, and meal preparation to maintain a safe living environment Maintain strict confidentiality of patient information in compliance with HIPAA regulations while communicating effectively with clients and healthcare teams Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly preferred Demonstrated knowledge of medical terminology and familiarity with home health documentation practices Experience working with individuals with developmental disabilities or geriatrics is advantageous Certification in First Aid and Basic Life Support (BLS), including CPR training required; additional certifications such as hospice or dementia care are a plus Proven ability to handle heavy lifting safely and operate patient transfer equipment like Hoyer lifts or similar devices Knowledge of home health agency operations and familiarity with EMR/EHR systems for accurate medical records management Strong communication skills to effectively engage with patients, families, and healthcare professionals while maintaining patient confidentiality Join us in delivering compassionate home health care that empowers individuals to live healthier, happier lives.
