Job Description

Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our healthcare team. In this role, you will provide essential nursing care to diverse patient populations across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. The ideal candidate will possess a strong foundation in patient assessment, medication administration, and clinical procedures, ensuring the delivery of high-quality, patient-centered care. This position offers an opportunity to make a meaningful difference in patients' lives while working within a supportive and professional environment. Duties Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans. Administer medications and injections accurately following prescribed protocols, including IV infusions and immunizations. Assist with diagnostic evaluations such as specimen collection, laboratory testing, and medical imaging procedures. Monitor patients' conditions closely, recognize signs of acute pain or deterioration, and escalate concerns appropriately. Provide specialized care for patients with complex needs such as dementia, Alzheimer's disease, or developmental disabilities. Support patients during procedures such as catheterization, tube feeding, wound suturing, and ventilator management. Maintain detailed medical documentation within electronic health records (EHR/Epic systems), ensuring compliance with HIPAA and confidentiality policies. Educate patients and their families on health management strategies, medication regimens, and post-discharge care instructions. Collaborate with multidisciplinary teams including physicians, case managers, therapists, and caregivers to coordinate comprehensive care. Ensure infection control protocols are rigorously followed to maintain a sterile environment.
